Care Management Concierge & Home Care Agency

Are you a dynamic RN leader ready to elevate care management to the next level? Lifespan is looking for a strong, solutions-driven Director of Care Management to lead, mentor, and support our team of nurses and social workers while ensuring seamless, high-quality client care.

What You'll Do

Lead & Empower the Care Management Team
  • Supervise, mentor, a team of care managers; nurses, and social workers.
  • Provide clinical oversight, case review, and decision-making support for clients aging needs.
  • Establish clear workflows, best practices, and quality standards to ensure premium care.

Delegate & Optimize Operations
  • Streamline care management processes to improve efficiency and team productivity.
  • Oversee case assignments, documentation, and compliance to maintain top-tier service.
  • Ensure care managers have the training, tools, and support they need to excel.

Client Onboarding & Care Planning
  • Conduct comprehensive assessments and care planning for new clients.
  • Collaborate with families, caregivers, and medical providers for seamless coordination.

Develop Policies & Drive Growth
  • Refine and enhance policies, procedures, and care management strategies.
  • Work alongside leadership to improve services and expand impact.

Qualifications

Who You Are

What You'll Need
  • Active RN license in the state of California.
  • 5+ years of leadership experience in care management or home health.
