Job Description:

- Assess patient conditions and implement care plans.
- Provide ongoing education and support to patients and their families.
- Deliver patient care in alignment with the organization's philosophy, policies, and community health practice standards.
- Perform admission processes and manage cases as per home care policies and standards of practice.
- Utilize thorough and timely electronic documentation.
- Maintain interdisciplinary communication to ensure optimal patient care.
- Participate in weekend rotations once every 6 weeks.
