Job Title: Registered Nurse
Job Specialty: Home-Hospice Care Operations
Job Duration: 13 weeks
Shift: 8-hour Day Shift
Guaranteed Hours: Not specified
Experience: 1 year of Home Health Registered Nurse experience
License: Active RN license
Certifications: Active Basic Life Support (AHA)
Must-Have:
- Reliable transportation
- Active driver's license
- Insurance
- Wound care experience (e.g., wound vacs, pleural drains, ostomy care)
- Ability to complete peripheral blood draws, central line care, and IV administration through multiple devices (e.g., elastomeric, IV pumps)
- Excellent assessment skills
- Experience with computer documentation
Job Description:
- This position is fully field-based, requiring travel from one patient to the next. There is no physical office, with all meetings conducted virtually, and nursing supplies delivered.
- Initial onboarding is required on-site for 1-2 days.
- The Case Manager will see 20-30 patients weekly, with the expectation of 6 routine visits per day.
- Patients typically have multiple comorbidities, including (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, falls, fractures, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.
- Care includes education on medications, disease management, and self-care, along with performing skills such as wound care, infusion, line maintenance, ostomy care, and teaching patients how to perform these care tasks.
- EPIC/Computer charting and Sapphire IV pumps experience are used.
- The traveler will be provided with a laptop for charting.
- Any solid color scrubs are acceptable.
- Must provide own transportation for patient visits.
- Coverage area is to be determined.
