Overview
$5,000 sign-on bonus and
$5,000 relocation (for greater than 100 miles) or
$2,500 relocation (for greater than 50 miles)
for nurses with 1 or more years experience - External Applicants Only
Coordinates outpatient care of the Maternal Fetal Medicine patient population within the MFM Department. Provide clinical expertise and management for the out-patients within the MFM program. Serves as a liaison and resource to patients, staff members, as well as provides direct patient care.

Responsibilities
Coordinates patient care activities and transitions of care for patients with the interdisciplinary team through the longitudinal care processes of MFM valuation and management in partnership with providers (including processes of assessment, planning, treatment, scheduling, internal/external referral, and follow up).
Coordinates follow up appointments through the treatment and surveillance care continuum.
Facilitates appropriate follow up of diagnostic results.
Initiates and updates individualized patient centered plans of care.
Documents the nursing plan of care and associated care coordination and transition management in the medical record.
Delivers holistic patient care in the form of direct care, psychosocial support, and education.
Participates in the development and delivery of patient care and longitudinal plan of care.
Promotes the ethical care delivery through professional nursing practice including patient advocacy, coaching and counselling.
Develops and maintains comprehensive educational materials relative to the continuum of MFM patient care.
Provides educational material and resources to patients within the comprehensive MFM setting.
Uses evidence based practice to promote patient / family / caregiver engagement, activation and empowerment.
Participates in community outreach programs.
Role modeling through the prioritization of activities and the setting and attaining of professional, personal, and organizational goals.
Demonstrates efficient management of time and resources.
Demonstrates the ability to assess, problem-solve, and critically evaluate unique circumstances to make evidence based decisions.
Coordinates and attends appropriate organizational and departmental meetings where employment related matters, policies, procedures, and programs are discussed. Maintains leadership role as appropriate within division meetings.
Collaborates with the provider group and administrative leadership to develop practice guidelines, including processes of care, care protocols, policies and procedures.
Develops and maintains division-specific documentation tools in collaboration with IT resources.
Collaborates with Supervisor Care Coordinator as it relates to Patient Care Conferences.
Collaborates with leadership to identify and report key operational and programmatic metrics.
Demonstrates positive communication skills with patients, families and co-workers.
Promotes positive communication in peer interactions.
Communicates effectively with the interprofessional team to effectively and efficiently provide high quality patient care.
Coordinates as indicated through the treatment plan.
Promotes the development and delivery of evidence-based practice.

Compensation and Benefits
The compensation range for this position is $30.44per hour - $52.36per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
