The RN Navigator is an experienced registered nurse who is provides timely clinical care coordination of patients within the ambulatory neurosurgery clinic setting. The navigator works in collaboration with patients, the care team and other interdepartmental colleagues to establish and coordinate patients plans of care across the UR Medicine system.

Responsibilities:
GENERAL PURPOSE:
The RN Navigator is an experienced registered nurse who is provides timely clinical care coordination of patients within the ambulatory neurosurgery clinic setting. The navigator works in collaboration with patients, the care team and other interdepartmental colleagues to establish and coordinate patients plans of care across the UR Medicine system.
ESSENTIAL FUNCTIONS
- Serve as the initial point of contact for patient referrals to Neurosurgery.
- Review urgent referrals for acuity and facilitate communication directly with referring office for appropriate scheduling of consultations.
- Work collaboratively with the Neurosurgery Referral Coordinator to efficiently process referrals by following and applying standard policies and practices.
- Serve as the liaison between the neurosurgery attendings, Advanced Practice Providers, Administrative staff, referring providers, clinic nursing staff, and regional (outreach) care locations.
- Determines if preliminary testing (imaging) is needed based on office practice and procedures. Responsible for explaining to referring provider and patient for completeness prior to visit.
- Provide education about diagnosis, care team role, side effect management and importance of treatment adherence.
- Develop and update appropriate written patient education materials in collaboration with the health care team.
- Create an environment in which staff learning is supported and stimulated.
- Assist the APP team with post discharge phone calls to all neurosurgical patients within 3 days of hospital discharge. Provide compliance data for department for meeting this hospital standard.
- Serve as a resource liaison for staff in other departments within the UR Medicine system and Primary Care Providers.
- Facilitate communication among members of the multidisciplinary care team.
- Participate in continuous quality improvement initiatives and assumes responsibility for their professional development.

QUALIFICATIONS
- Bachelor's degree in Nursing (BSN) required.
- 3 years of Nursing experience required or equivalent combination of education and experience required.
- Competence in critical thinking, interpersonal relationships and technical skills required.
- A strong working knowledge of neurology, neurosurgery and/or ortho spine preferred.
- A strong working knowledge of neurology, neurosurgery and/or orthopedic spine preferred.
- Excellent organizational and time management skills with attention to detail preferred.
- Knowledge and experience documenting patient care in an electronic medical record preferred.
- Excellent verbal, written and presentation skills preferred.
- Ability to make knowledgeable clinical assessments and judgements preferred.
- Ability to educate staff and families on navigating the UR Medicine system preferred.
- Ability to communicate effectively with all members of the health care team and external contacts using tact, discretion, and diplomacy preferred.
- Well-developed problem solving and team development techniques preferred.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure upon hire required.
- Certification in Nursing specialty area upon hire preferred.
