SALARY RANGE: 74,838.40 - 122,324.80YEARLY
General Summary:
To provide direct patient care and comprehensive coordination of care for selected patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, increase patient and provider satisfaction, maintain point of contact with providers and serve as the first point of contact for patients and families. Navigate and collaborate with other members of the patient care team to direct and evaluate delivery of patient care across the care continuum. Collaborate with patients, primary care providers, surgeons and other sub specialties on the provision of comprehensive care plans
Responsibilities:
1. Navigates the patient through their treatment plan providing support and resources to improve patient outcomes. Central point of contact for all navigated patients; including interaction with medical, nursing, pharmacy, ancillary services, and post-acute care organizations for up to 90 days post discharge. Monitors data and manages quality outcome data and compiles in reports to the senior director and medical leadership
2. Meets with disease specific/selected newly diagnosed patients to collaborate on medical management and/or interventional processes. Reinforces education to the patient and family regarding the disease process, diagnostic and interventional procedures, tests, labs, medications, and the goals of the treatment regimens. Track order completion and follow up. Monitor patient compliance with patient treatment plan.
3. Performs follow up calls on selected patients to encourage adherence to established discharge and rehabilitation plans for up to 90 days post discharge. Assures care is coordinated and timely by following up with patient to initiate plan of care. Assist patients with referrals to outside resources and care coordination as needed. Works with hospital engaged third parties as necessary to ensure PAC coordination.
4. Advocates for best practice, guidelines-driven patient care. Develops protocols as directed. Mentors and provides feedback and coaching through a formal plan to assist in developing other key stake holders. Leads workgroups to achieve desired clinical outcomes and targeted goals. Works with other navigators as necessary.
5. Systematically evaluates effectiveness of the navigation using data driven quality measures. Collect and report data from patient on barriers, health care, medications, prior treatments, etc.
6. Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
7. Assists with the development of care plans that prescribe specific interventions or life style modifications to attain expected outcomes.
8. Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives and safety, environmental, and/or infection control standards.
9. All other duties as assigned within the scope and range of job responsibilities

Required Education, Credential(s) and Experience:
- Education: Bachelor Degree
; Nursing
; or Graduate of Accredited School of Nursing with 4 years specialty area nursing, care management / patient navigation experience in lieu of BSN. - Credential(s): Registered Nurse
Basic Life Support
; - Experience: Required: 2 years clinical specialty area nursing setting, care management / patient navigation, or related physician practice experience. Preferred: Previous patient care coordination experience.
Preferred Education, Credential(s) and Experience:
- Education: Master Degree
Nursing - Credential(s): Certification related to the position, if applicable.
