Description:
The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care.
Care is based on nursing theory and research for a designated group of patients and their significant others.
Delivers patient care by incorporating the tenets of Relationship Centered Care.
Utilizes the nursing process based upon theory, research, evidence-based practice, and approved organizational standards.
Assesses and evaluates patient care needs and applies critical thinking in care management.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in developing, implementing, and evaluating the plan of care.
Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after care, delegating to others as appropriate.
Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease.
Communicates effectively and professionally with patients, families, and all members of the care team.
Manages assignments within the modified primary nursing delivery model.
Role models the tenets of Relationship Centered Care.
Addresses patient values, preferences, and cultural diversity in all aspects of care.
Establishes and maintains a therapeutic relationship with both patients and their families.
Uses evidence-based practices to understand patients' perceptions of care.
Initiates service recovery for patients and families who have concerns and escalates as needed.
Demonstrates caring, respect, compassion, empathy, and active listening through dialogue and actions.
Conducts hourly rounding on assigned patients to address their needs.
Practices safe and efficient patient-centered care while identifying barriers and opportunities for improvement.
Demonstrates clinical skills crucial to quality and safety in the served patient population.
Implements process improvement strategies based on institutional and evidence-based procedures.
Engages in peer and institutional review processes.
Collaborates with teams to create and implement quality improvement projects and nursing research studies.
Contributes to quality and safety practices.
Implements principles to avoid high-risk behaviors providing a safe environment for patients.
Escalates potential safety hazards following established protocols.
Documents nursing practice accurately to support quality and performance improvement initiatives.
Performs charge nurse duties as assigned.
Coordinates with management to ensure supplies are utilized appropriately and inventory is sufficient.
Escalates resource deficits to management as needed.
Demonstrates patient-focused and cost-effective approaches to care.
Facilitates patient throughput on a shift basis.
Contributes to an environment of care supporting clinical colleagues, patients, and families.
Advances clinical competence in nursing practice.
Projects a professional image and communicates in ways that demonstrate caring and respect.
Incorporates ethical principles into decision-making for patients and families.
