Job Description
Job Title: Registered Nurse (RN) Case Manager
Job Specialty: Case Management/Discharge Planning
Job Duration: 13 weeks
Shift: 8-hour days, Monday to Friday (8:00 AM - 4:30 PM)
Guaranteed Hours: 40 hours per week
Experience: Minimum 2 years of Acute Care Management experience, including at least 6 months in a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge.
License: RN license required, willing to wait for processing.
Certifications: Basic Life Support (BLS) required; Certified Case Manager (CCM) preferred but not mandatory.
Must-Have:
- EPIC Charting experience
- General understanding of discharge readiness
Job Description:
- Perform case management and discharge planning for all inpatient units.
- Manage a caseload of 15-20 patients/consults per day.
- Collaborate with a team of 15 full-time and 17 part-time social workers.
- Work with social discharges and medical discharges with complex placements.
- Utilize contracts with local facilities to manage complex patient placements.
- Report directly to the Social Work Manager, with oversight from the Chief of Care Coordination as needed.
- Complete a 2-week orientation, including general facility orientation.
- Maintain communication with Respiratory Therapy and Pharmacy for discharge planning as needed.
- Rotated holiday requirements.
- No weekend requirements.
- Flexibility in dress code, allowing any color scrubs or business professional attire.
Additional Information:
- Orientation will ensure you work alongside the Case Management team and not independently.
- Be prepared to think creatively for complex patient placement solutions.
- A supportive team environment with a goal to expand the Case Management team.
