Shift: 8-hour days, Every Other Weekend
Guaranteed Hours: 40 hours per week
Experience: Minimum of 2 years
License: CA RN License
Certifications: BLS
Must-Have: Prior experience in Care Coordination
Job Description:
- Manage and coordinate patient care to ensure effective use of resources and high-quality healthcare services.
- Collaborate with interdisciplinary teams to evaluate patient needs and develop care plans.
- Utilize the Cerner charting system for accurate and efficient documentation.
- Participate in case reviews and contribute to the development of strategies for optimal patient outcomes.
- Uphold hospital and unit policies, with flexibility to adapt to changing patient and unit needs.
- Participate in a mandatory 12-hour orientation program prior to shift commencement.
- Comply with a cancellation policy that allows cancellation of up to 4 shifts during the assignment.
- Maintain clear and professional communication with healthcare teams and patients.
- Ensure adherence to federal and state regulations in healthcare provision.
