Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Care Management Team! In partnership with the Senior Manager of Patient Programs, the Senior Care Management Program Coordinator is responsible for the Care Management program at Erie and other population health management initiatives, including but not limited to agency-wide transitions of care and ED navigation efforts. The Senior Program Coordinator utilizes their clinical knowledge and program coordination skills to ensure that these programs and activities are implemented and optimized in line with both contractual requirements and Erie's standards for high-quality, patient-centered care. The Senior Program Coordinator leads the integration of these activities at the site levels with the care teams and across the Patient Programs department and other centralized teams at Erie.

Main Duties & Responsibilities
Leadership & Program Coordination Responsibilities (approximately 60% time)
Team Leadership
- Supports and mentors direct reports in staff support and performance management
- Oversees new hire onboarding and ongoing team trainings
- Provides consistent communication through regular team meetings, direct report 1:1s, and cascading sponsorship of agency priorities and communications
Program Coordination & Process Improvement
- With input from Patient Programs leadership and Sr. Manager of Patient Programs, lead development, documentation, and dissemination of training and workflows for care management and other assigned activities across all Erie sites.
- In coordination with the Senior Manager, supports the development and revision of Standardized Operation Procedures (SOPs) and workflows to support program structure, staffing productivity, and contractual requirements.
Quality Assurance
- Assists Manager in ensuring initiatives meet grant, contractual, and organizational goals and objectives by validating accuracy of data, conducting appropriate analysis and reporting variances to manager
- Ensures program completion of all required documentation.
- In coordination with the Senior Manager, Patient Programs evaluates Medical Home Network (MHN) requests, reports, and resources to optimize Erie's performance and ensure integration with Erie systems and structures.
Direct Service Responsibilities (approximately 40% time)
- Provides care management services, including completion of comprehensive risk assessment, care plans, medication reconciliation, and monthly follow-ups for a panel of patients, consistent with contractual and Erie requirements for quality and service excellence
- Conducts chart reviews, utilizes electronic medical records to identify patients' clinical concerns and acuity, risk factors, and barriers when enrolling patients to program.
Qualifications
Education
Required
- Bachelor's degree with 3 years nursing experience OR Associates degree with 6 years nursing experience required
- Current Illinois RN licensure required
Preferred
- Master's degree preferred
- Formal leadership, change management, or quality improvement training preferred
Experience:
Required:
- Three (3) years of nursing experience required
- One (1) year of supervision experience required
Preferred:
- Program coordination or process improvement experience preferred
- Experience working in community health or another safety net setting preferred
- Three (3) or more years of direct nursing experience as a care manager or nurse leader preferred
- Managed care experience preferred
- Additional supervision experience preferred
Skills and Knowledge
Required:
- Ability to establish and maintain effective working relationships with co-workers, clients, and community partners
- Ability to plan, organize, implement, evaluate, and modify program needs
- Ability to problem solve and address issue as they arise
- Ability to work with staff members across all levels of the agency
- Strong computer skills: expert in Microsoft Office, Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system, etc.).
- Active CPR card required
Preferred:
- Bilingual fluency (English/Spanish) a plus. (May be required to assist or work with non-English/Spanish-only speaking patients or guests.)
