Job Title: Registered Nurse (RN)
Job Specialty: Case Management

Experience: Minimum of 2 years in hospital case management
License: Must have a Texas or multi-state RN license
Certifications: Basic Life Support (BLS) certification through the American Heart Association
Must-Have: RN Case Management and discharge planning experience, along with 2 years of recent hospital case management experience
Job Description:
- Coordinate clinical and psycho-social activities with the interdisciplinary team and physicians to provide comprehensive care coordination and discharge planning for each patient.
- Utilize critical thinking to develop and execute effective discharge planning.
- Promote effective and efficient utilization of clinical resources.
- Monitor all areas of patient's stay for effective care coordination and efficient care facilitation.
- Conduct medical necessity reviews for appropriate utilization of services from admission through discharge.
- Document all patient review records according to regulatory, legal, and coordination requirements.
- Perform reviews for third-party payors as appropriate.
- Stay current on reimbursement modalities, community resources, and case management or psychosocial legal issues affecting patients and providers.
- Conduct comprehensive, ongoing biopsychosocial assessments of patients and families to provide timely and safe discharge planning.
- Refer cases where patients and families would benefit from additional resource support to a social worker.
- Serve as a patient advocate and enhance collaborative relationships to maximize patient and family decision-making.
- Participate in interdisciplinary patient care rounds and conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-hospital needs.
- Collaborate with clinical staff in developing and executing the plan of care and achieving goals.
- Demonstrate knowledge of the principles of growth and development over the lifespan and the skills necessary to provide age-appropriate care to the patient population served.
- Coordinate social services for patients, families, and significant others to manage the impact of illness and achieve maximum benefits from healthcare services.
- Serve on division committees when requested.
- Initiate patient and family education and discharge planning.
- Arrange for discharge and post-hospital care of patients through community institutions and agencies.
