Job Description:

- Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians to provide comprehensive care coordination and discharge planning for each patient.
- Utilize critical thinking to develop and execute effective discharge planning.
- Promote effective and efficient utilization of clinical resources. Monitor all areas of patient stay for coordination and efficient care facilitation.
- Conduct medical necessity review for appropriate utilization of services from admission through discharge.
- Document all patient review records according to regulatory, legal, and coordination requirements.
- Perform reviews for third-party payors where appropriate.
- Maintain an up-to-date knowledge base regarding reimbursement modalities, community resources, and case management or psychosocial legal issues that affect patients and care providers.
- Conduct job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and professional standards.
- Conduct comprehensive, ongoing biopsychosocial assessments of patients and family for timely and safe discharge planning.
- Refer cases where patients and/or family would benefit from additional resource support to the Social Worker, if applicable.
- Serve as a patient advocate and enhance collaborative relationships to maximize patient and family decision-making capabilities.
- Participate in interdisciplinary patient care rounds and/or conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-hospital needs.
- Collaborate with clinical staff to develop and execute the plan of care and achievement of goals.
- Demonstrate knowledge of growth and development principles over the lifespan and the skills necessary to provide age-appropriate care to the patient population served.
- Coordinate the provision of social services to patients, families, and significant others to manage the impact of illness on family functioning and achieve maximum benefits from healthcare services.
- Serve on Division committees when requested.
- Initiate patient and family education and discharge planning upon admission.
- Arrange for discharge and post-hospital care of patients through community institutions and agencies.
