Job Title: Skilled Nursing Physical Therapist
Job Specialty: Therapy - Physical Therapy
Job Duration: 13 weeks, option for extension
Shift: Day - 8x5 - 08AM
Guaranteed Hours: 36 hours per week
Experience: Open to New Graduates
License: Must be licensed in the state(s) of practice as a Physical Therapist
Certifications: Department-specific competencies and certifications as required
Must-Have:
- Licensure in the required state(s)
- Valid driver's license if applicable
Job Description:
- Plan, organize, and deliver physical therapy programs to help clients/patients improve mobility, relieve pain, increase strength, and prevent deformities.
- Conduct examinations, evaluations, and interventions for patients affected by injury or disease.
- Administer physical therapy interventions using appropriate tools and techniques; ensure patient safety during treatments.
- Document client/patient information for evaluation; establish and modify therapeutic plans as necessary.
- Educate clients/patients and their families about treatment plans and home exercises.
- Apply knowledge of age-related treatments and evaluations to address the needs of clients/patients.
- Incorporate current research into daily treatment plans for clients/patients.
- Focus primarily on post-acute, long-term care, and home health care environments.
- Maintain documentation according to departmental guidelines and meet regulatory requirements.
- Demonstrate specialized treatment expertise to clients/patients.
- Follow clinical practice guidelines of the professional organization.
- Exhibit flexibility in accepting variable work schedules/assignments.
- Participate in activities promoting the department and facility.
- Collaborate with individuals outside the department, including providers, case managers, and others.
- Supervise assistants, aides, and students providing patient care or other services.
- Provide ongoing education and training to peers as determined by department leadership.
- Establish and maintain productive working relationships within and outside the department.
- Identify and address common patient safety risks.
- Analyze and record therapeutic progress and recommend necessary adjustments.
- Evaluate and modify treatment plans according to client/patient health status.
- Ensure compliance with legal regulations and organizational policies when discussing patient health information.
- Seek mentorship opportunities and demonstrate self-motivation.
- Recognize ethical requirements in specialized areas of responsibility.
- Adhere to safety procedures and standards during therapy processes.
- Software used: Netsmart for home health and Net Health for skilled nursing facilities; training available.
- Flexibility in scheduling; normal is Monday-Friday, but adjustments can be made.
- Productivity expectations include evaluations and group/concurrent time.
