Shift: Day 5x8-Hour (07:30 - 16:00) Monday - Friday
Guaranteed Hours: Contracted Hours
Experience: Experience with Inpatient Hospital Medical Social Work, Discharge Planning, EPIC Experience
License: MN State License
Certifications: 4 Year Degree in Social Work
Must-Have:
- MN State License in hand
- Experience with inpatient hospital medical social work
- Discharge planning experience
- EPIC experience
Job Description:
- The Licensed Social Care Worker will accept LSW with a Bachelor's degree.
- The candidate will work in an acute care hospital setting that has a total of 159 staffed beds and operates as an Adult Level III Trauma center.
- Responsible for providing comprehensive social work services, including discharge planning.
- Required to wear appropriate scrub color as mandated by the department.
- Must comply with facility vaccine policies, with only medical or religious exemptions considered.
- Participation in orientation for 14 hours is necessary.
- Complete fingerprinting in person as stipulated by state requirements.
- Engage in effective communication and collaboration with multidisciplinary teams to facilitate excellent patient care.
- Handle any holiday work requirements as needed by the facility.
