This position provides training/consultation/support for and quality monitoring of case managers in contracted agencies and within Social Services.
Minimum Qualifications

NEED TO HAVE QUALIFICATIONS
  • Bachelor's degree in Social Work, Sociology or related field AND
  • Three years professional work in a Social Services or Public Health agency working with mental health case management and/or home community-based waivers and/or
  • Valid Driver's License

NICE TO HAVE QUALIFICATIONS
  • Masters degree in Health or Human Services (e.g. nursing, social work, sociology, or related field)

Duties & Responsibilities

These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position's management approved work schedule is required for all positions.

"Case Managers" refers to contracted agency staff responsible for delegated contracted case management and Social Services' case managers as delegated by management:

1. Effectively engage with case managers to provide effective person-centered services.
2. Provide training, consultation, support and information regarding programs' policies, procedures and standards to case managers.
3. Coordinate and monitor quality outcomes of case management services by documentation of satisfactions surveys; audits; incident reports; and peer reviews.
4. Develop/maintain effective operational transitions and linkages to: assure effective/efficient transfer to case manager, timely client connections, compliant work flow and documentation per program, and effective linkage with core community and County supports (e.g. crisis, VA/Child protection, conservator/guardian, health systems, providers, etc.)
5. Provide timely case management consultation for urgent clinical or administrative needs. Prioritize and address with management if additional consults needed to resolve issue.
6. In accordance with delegated authority (scope defined by Social Service' Management) processes/reviews/approves/denies requests for Purchase of Services. Case Management Coordinator assures consistency of documentation /processes from all stakeholders.
7. Organize operational meetings and trainings between County and agencies to address inter-agency compliance with fed/state/county/provider standards and processes.
8. Document key deliverables provided by case management agency quarterly and/or annually in format determined by management team.
9. Demonstrates a commitment to support cultural diversity County wide, actively supports the County's diversity initiatives, and works to address the needs of diverse and underserved communities.

Knowledge, Skills & Abilities and Work Environment

  • Knowledge of federal, state, and local laws, rules, regulations, and policies pertaining to case management responsibilities
  • Knowledge of systems, resources, and programs that support people who access DD, Mental Health, Chemical Health and other disability-related services
  • Knowledge of targeted case management and waiver services.
  • Knowledge of crisis placements, crisis respite and crisis providers that serve individuals with disabilities
  • Comprehensive knowledge of computer software and information systems.
  • Ability to communicate effectively orally and in writing.
  • Ability to investigate problems, set a plan for mitigating, and implement.
  • Ability to work independently and as team member.
  • Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors, including individuals with a disability or whose first language may be other than English.

WORK ENVIRONMENT: Work is performed remotely, in a county office building, and contracted case management agencies' buildings. Travel is required between these entities. Lifting requirement up to 20 pounds on occasional basis. Equipment used may include, but not limited to, computer, printer, telephone, copy/fax/scan machines.
