Job Description:
- Fulfill the needs of patients, families, and the community by providing social work services in accordance with licensure, certification, training, and regulatory requirements.
- Assume responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit.
- Conduct discharge planning, coordination of care, and other planning and counseling activities to effectively move patients through the continuum of care in a cost-effective and resource-efficient manner.
- Respond effectively to new and changing work assignments.
