The Opportunity
Position Summary:
Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services to assist with the development of the discharge plan. Communicating with the client and family about progress toward the appropriate discharge location.
Program Summary: Complex Care - for frail seniors who require the care of an interdisciplinary team to maintain and improve quality of life in safe, comfortable and supportive environments. Carewest offers a mix of secured and unsecured units.
Qualifications & Experience:
- Graduation from a recognized degree program in Social Work or equivalent program Registered with Alberta Association of Social Workers;
- Driver's License
- Minimum of 2 years experience in a combination of clinical and continuing care roles;
- Gerontology experience is preferred
