Job Title: Social Worker

Guaranteed Hours:

Experience: Minimum of 2 years

License: Current LCSW or LICSW license in Massachusetts

Certifications: Basic Life Support (BLS)

Must-Have: Skilled nursing facility or rehab experience preferred

Job Description:

- Assess patients and families, providing intervention and treatment to support patients.
- Address psychosocial needs and patterns of adjustment throughout hospitalization.
- Collaborate with the multidisciplinary team to formulate treatment plans that maximize emotional, social, and physical well-being.
- Provide information and education regarding health care and community resources.
- Complete the Minimum Data Set (MDS) in conjunction with the interdisciplinary team.
