Job Description:
- Work with adult patients in a home care setting.
- Manage a caseload of 3-4 patients per day, including a combination of evaluations and subsequent visits.
- Engage in discharge planning and provide necessary support to patients.
- Familiarity with EMR system HomeCare Home Base is preferred, though department-specific orientation is available for those without experience.
- Participate in meetings twice a month to ensure all documentation, including personal timesheets, is properly approved and recorded.
- Dress code is business casual.
- Holiday coverage may be required, specifically minor holidays.
- No weekend requirements.
- Travel within designated territory as specified on the provided map. Mileage is billable per guidelines.
- Participate in Department Specific Orientation, potentially up to two weeks if no prior experience in HCHB EMR.
