Description
Hennepin Health is looking for a manager to support both internal and external care coordination programs within the Special Needs Basic Care (SNBC) team. This dynamic leader will organize activities to support health care equity and the provision of appropriate social services to Hennepin Health members who are certified disabled. The SNBC Manager will develop policies and procedures regarding care coordination in accordance with the Department of Human Services (DHS) Medicaid requirements and lead their implementation in partnership with other Hennepin Health staff and Hennepin County Human Services waiver case management staff.
Hennepin Health provides health care coverage to Hennepin County residents who are enrolled in Prepaid Medical Assistance (PMAP), MinnesotaCare or SNBC. Our mission is to improve the health of Hennepin County residents through innovative collaboration with health care providers, Hennepin County services, community organizations and Hennepin Health members by changing how we build healthy equitable communities. The SNBC manager leads the development and execution of care coordination program goals and objectives in collaboration with other areas within Hennepin Health and with external subcontracted care coordination agencies.
Location and hours:
This position is hybrid and will be performed both on-site at the Health Services Building, 525 Portland Avenue South, 8th floor, Minneapolis, MN 55415, and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In this position, you will:
- Oversee a dynamic, interdisciplinary team of professionals fostering a positive, engaging workplace culture where staff are open to new approaches to improve efficiency and positive impact to members.
- Oversee the development and execution of care coordination goals, objectives and initiatives to meet operational requirements, compliance with policy guidelines, regulatory and contractual requirements, internal and external audits, outcome reporting, and best practices.
- Direct the development of a care coordination training program for staff; establish standards for quality of work; develop outcome and utilization measures; supervise and evaluate employee performance.
- Develop and lead the implementation of care coordination policies and practices provided by care coordination teams to Hennepin Health members as required by Hennepin Health's contracts with DHS, county policy, state and federal regulation.
- Organize and coordinate member-facing care coordination activities, promoting collaboration across Hennepin Health, county departments, provider partners, and other external organizations to develop coordinated processes to achieve improved outcomes for SNBC members.
- Direct the development of budgets and expenditure tracking; advocate resource allocation to care coordination activities including SNBC member-facing benefits, innovative programming, and internal costs.
- Provide leadership to further department-based strategies that align with county goals, mission and core values for disparity reduction and workplace inclusion.
