Must-Have: Proficient computer skills with the ability to learn new software applications, experience working in an interdisciplinary team, strong verbal, written, and electronic communication skills, excellent interpersonal and customer service skills, ability to work with a diverse population
Job Description:
- Provide speech therapy services to patients in their residence with impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services.
- Manage technical, environmental, and financial resources efficiently.
- Conduct age-appropriate evaluations reflecting patient history, systems review, and necessary tests and measures.
- Participate in weekend rotations once every six weeks.
- Reassess treatment plans at least every two weeks, and coordinate with patients, caregivers, and interdisciplinary teams to meet discharge goals.
- Ensure physician orders are current and consistent with the plan of care, documenting them in the medical record.
- Submit patient schedules to the rehab scheduler by the end of the week.
- Orient and supervise home health aides according to agency and state guidelines.
- Manage patient caseloads in line with agency, state, and professional guidelines.
- Maintain timely and complete documentation according to agency standards.
- Update POC devices weekly as required.
- Ensure verification of payor information on admission/evaluation within agency and regulatory guidelines.
- Maintain and transmit documentation supporting managed care authorizations/DME requirements within designated timeframes.
- Ensure visit schedules align with managed care payor authorizations.
- Discuss utilization and caseload with the Rehab Manager to make necessary changes to reach goals.
- Achieve reportable outcomes that meet agency goals.
- Identify realistic, objective, time-oriented goals for patient performance based on examination findings.
- Develop treatment and discharge plans based on examination results and patient goals.
- Inform patients of their weekly schedule and any necessary changes.
