- Excellent interpersonal and customer service skills with the ability to work with a diverse population
Job Description:
- Provide speech therapy services to patients in their place of residence who have impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years.
- Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
- Age-appropriate evaluations are completed that reflect patient history, relevant systems review, and appropriate tests and measures, and reported to primary clinicians.
- Weekend rotations occur once every 6 weeks.
- Reassess the plan of treatment at least every 2 weeks with patients, primary caregivers, and the interdisciplinary team; modify and communicate the plan as appropriate to meet discharge goals.
- Ensure physician orders are current and consistent with the plan of care and documented in the medical record.
- Submit patient schedules to the rehab scheduler by end of day Thursday.
- Orient and supervise home health aides in accordance with agency and state guidelines.
- Manage patient caseloads in accordance with agency, state, and professional guidelines.
- Complete documentation and export timely according to agency standards.
- Update point-of-care devices weekly and as directed.
- Complete verification of payor information on admission/evaluation within agency and regulatory guidelines.
- Maintain and transmit documentation to support managed care authorizations/DME requirements within designated time frames.
- Ensure the visit schedule is consistent with authorizations by managed care payors.
- Discuss utilization/caseload with the Rehab Manager on an ongoing basis and make changes toward stated goals.
- Ensure reportable outcomes meet agency goals.
- Identify realistic, objective, and time-oriented goals for patient performance based on examination findings.
- Select treatment and discharge plans based on examination findings and patient goals.
- Inform patients of their weekly schedule and any necessary changes.
