SUMMARY
The Housing for Health Division (HFH) at the Los Angeles County Department of Health Services provides a continuum of services for people who are experiencing homelessness and who have complex health or behavioral health conditions. HFH seeks a mission-oriented, process-minded analyst to support its programs and teams.
HFH, in partnership with LA DHS Rancho Los Amigos National Rehabilitation Center, is seeking a Staff Analyst to join the Mobile Clinics team to support programing in Service Planning Areas (SPAs) 1, 2, 4 and 5. The mission of the Mobile Clinic is to provide low-barrier access to clinical services for unsheltered people experiencing homelessness in LA County. The Staff Analyst works closely with the Mobile Clinic Senior Staff Analyst and Medical Director to support program development, including by not limited to provision of patient services to unsheltered PEH exiting emergency departments and inpatient hospital settings, community engagement and technical assistance efforts with county-operated and local hospitals, and care coordination activities that support Mobile Clinic goals and objectives.
ESSENTIAL FUNCTIONS
Assists in coordination and collaboration with key stakeholders (e.g., county hospitals, field medicine teams, street outreach partners, medical clinics, etc.) to ensure all project elements operate in the most efficient and effective manner.
JOB QUALIFICATIONS
Four or more years of highly complex administrative or supervising experience in the health, housing, social services or related sectors.

Ability to work in-person daily at all Mobile Clinic locations. This position reports to Mid-Valley Comprehensive Health Center (eventually transitioning to Olive View Medical Center and High Desert Regional Health Center).
Field-based travel required.
Field activities, including meeting with partners at their locations/office settings.

ESSENTIAL FUNCTIONS
Assists in coordination and collaboration with key stakeholders (e.g., county hospitals, field medicine teams, street outreach partners, medical clinics, etc.) to ensure all project elements operate in the most efficient and effective manner.
- Provides technical assistance and educational opportunities to partners.
- Develops, recommends, and implements modifications to enhance care coordination of PEH exiting settings that require patient services (e.g., emergency departments, inpatient hospitalization settings, correctional settings, etc.).
- Assists with the evaluation of Mobile Clinic patient services and patient retention.
- Ensures collection of applicable demographic and programmatic data and tracks all patient data for quality assurance purposes and to provide regular reporting to funders and/or health plans.
- Assists the Senior Staff Analyst with monitoring grants and contracts.
- Assists with the coordination and facilitation of monthly all-staff educational meetings.
- Provides regional data analysis on metrics related to encounters, continuity of care, and other key performance indicators.
- Supports the operations and logistics for Contingency Management and other Mobile Clinic programming.
- Works with the Medical Director and Senior Staff Analyst to develop and implement new Mobile Clinic programming
- Responsible for implementing Transition of Care (TOC) support for County hospitals such as LA General Medical Center, Olive View Medical Center and Harbor UCLA Medical Center.
- Works directly with the Mobile Clinic staff for regular, daily in-person support needs.
- Executes PDSA cycles for continuous quality improvement, working with Behavioral Health and Nursing Supervisors on Clinical Quality measures
- Identifies gaps in workflow processes and suggests improvements.
- Supervises Mobile Clinic Heavy Truck Drivers.
- Supports the Mobile Clinic Assistant Staff Analyst and Drivers with fleet, supply, compliance, maintenance and facility operations.
- Performs other duties as assigned.
JOB QUALIFICATIONS
Four or more years of highly complex administrative or supervising experience in the health, housing, social services or related sectors.
Certificates/Licenses/Clearances
- Valid California Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
- Successful clearing through the Live Scan process with the County of Los Angeles

- Preference will be given to candidates who are knowledgeable of data systems utilized across DHS and the homeless system of care, e.g., CHAMP, ORCHID, Clarity, ArcGIS, Tableau, etc.
- Excellent written and verbal communication skills, including the ability to express technical concepts clearly to both technical and non-technical audiences.
