Cabell Huntington Hospital is looking for a dedicated Staff Pharmacist to join our team. In this role, you'll be part of a decentralized pharmacy model, which allows for more direct patient interaction and tailored medication management. Your focus will likely be on ensuring safe and effective medication use for a diverse patient population, collaborating closely with healthcare teams.

Shift / FTE: Full-time; Day Shift with Flexible Hours; typically, 8:00 a.m. - 4:30 p.m. based on clinical rounding of the team or other clinical responsibilities.

Key responsibilities may include:

  • Conducting medication therapy management including compounding and dispensing medications and supplies to inpatients and outpatients
  • Reviewing and verifying medication orders
  • Participating in patient rounds and interdisciplinary teams
  • Educating patients and staff about medications
  • Monitoring of medication administration through profile reviews, as well as active participation in clinical programs.
  • Ensuring compliance with pharmacy regulations and protocols
  • Responsible for dispensing of the following: 750 IV admixtures daily; 2,250 units of medications daily; 200 out-patient prescriptions.

Requirements

  • Education / Requirements
    • Bachelor's degree required, PharmD. preferred
    • WV Board of Pharmacy Licensure and registration required
