Job Description
Job Title: Registered Nurse (RN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day Shift, 8 hours per day
Guaranteed Hours: 40 hours per week
Experience: Minimum of 2 years of home health experience required, and at least 2 years of experience in medical surgical or skilled nursing facility if home health experience is not available. Travel experience is required.
License: Active and clear driver's license, car insurance, and Hawaii nursing license are required.
Certifications: COVID and Flu vaccines required. Homecare Home Base and OASIS knowledge required.
Must-Have: Bachelor's degree preferred. Case management experience preferred. Own transportation required.
Job Description:
- Provide excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program.
- Act as a liaison between the patient, family, nurse, physicians, multidisciplinary team, and healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items.
- Lead the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, and procedures, and other aspects of patient care as appropriate for acute care.
- Discuss with physicians the appropriateness of resource utilization, consultations, treatment plan, estimated length of stay, and discharge plan.
- Focus on complex patients, frequent emergency department utilizers, chronic pain, substance abuse patients, and homeless patients.
- Collaborate with acute care case managers to ensure appropriateness of ongoing care.
- Coordinate the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services.
- Provide oversight and collect data required for regulatory/accreditation compliance.
- Manage frequent emergency department visitors by conducting a focus study review of the previous and current admissions.
