Job Description:
- Provide assessment and intervention to assist clients and families in improving social and economic difficulties affecting health and wellness through the casework process, principles, strategies, and community resources.
- Deliver professional discharge planning services through assessments and coordination of post-hospital care needs to patients and their families, linking them to necessary emotional, medical, and spiritual care resources.
- Perform high-level triage of all patients, focusing on identifying those with complex psychosocial or financial issues, placement needs, and community service needs within 24 hours of admission.
- Receive referrals based on identified Social Work Triggers following standard operating procedures.
- Identify and assess barriers early in the patient's stay, formulating a plan with the patient, family, internal, and external members of the healthcare team, payers, and community resources.
- Assess patient and family needs for support and community services, such as Meals on Wheels and Sitters; educate and refer them to resources, arrange appointments, and establish rapport with other agencies.
- Assess the risk of readmission for specified patient populations and initiate interventions to enhance the patient's ability to successfully transition along the care continuum.
- Identify the need for, arrange, and participate in family care conferences; engage in interdisciplinary conferences and provide consultation for patients, families, and clinical staff.
- Identify and report avoidable delays and variances from established care plans to leadership.
- Identify patient and family preferences, needs, and strengths to support the interdisciplinary team in compliance with standards of care.
- Conduct interviews with patients and significant others to assess psychosocial situations and identify the family point of contact.
- Develop discharge plans in direct consultation with patients, families, physicians, and healthcare teams.
- Manage complex cases and intervene with and advocate for patients and families as care and discharge plans are developed.
- Utilize knowledge of insurance benefits and coverage guidelines to maximize appropriate utilization of resources.
- Document assessments, plans, interventions, barriers, and reassessments in the EMR to facilitate discharge and transitions; ensure pertinent information is transferred to post-acute agencies.
- Collaborate with RN Care Managers, other disciplines, and internal and external healthcare team members to ensure a safe, appropriate, and timely transition to the next care level, considering the patient's available resources.
- Partner with external agencies and facilities to provide continuity of care for patient and family empowerment and independence in making autonomous health decisions.
- Represent the integrated care management department on various teams and performance outcomes committees and projects.
- Perform other related duties as required and directed.
