Job Title: Home Health Therapist

Job Specialty: Occupational Therapy in Home Health

Job Duration: 13 weeks

Shift: Day Shift, Monday to Friday, 8:00 AM - 4:30 PM

Experience: Minimum 2 years required in Home Health

License: Valid Kentucky license required

Certifications: COVID-19 Vaccine and Booster required, Flu Vaccine required (Medical/Religious Exemptions Only)

Must-Have:

- Compliance with a 75-mile radius rule, with potential leniency by the Therapy department
- References required
- Candidates will not float within the hospital or to affiliate hospitals

Job Description:

- Provide skilled occupational therapy services in a home health setting
- Collaborate with a multidisciplinary team to ensure comprehensive patient care
- Conduct patient assessments and develop personalized care plans
- Deliver patient education and support for optimal health outcomes
- Comply with state and federal regulations and maintain professional standards
- Complete documentation requirements accurately and in a timely manner
- Attend a 16-hour orientation session
