Job Title: Physical Therapist
Job Specialty: Home Health Physical Therapy
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year Adult Home Health Experience
License: Must have a valid professional license in hand
Certifications: CPR, BLS
Must-Have: Reliable transportation, a valid driver's license, and liability insurance
Job Description:
- Conduct home visits for therapy sessions and participate in periodic office meetings.
- Manage an average of 5-6 patients per day, with a weekly productivity goal of 30-32 points.
- Must be familiar with HomeCare HomeBase EMR.
- Willing to travel up to 75 miles from the assigned clinic.
- Cover counties including Morgan, Brown, and Bartholomew.
- Use a point system where each visit and activity hold different point values.
- Flexibility required for potential weekend shifts approximately every 5 weeks.
- No on-call duties are required.
- Travel expected is approximately 250-300 miles per week, with mileage reimbursement at the IRS standard rate.
- Responsible for completing all documentation within 24 hours of patient appointments.
- Ability to float to nearby clinics if necessary to meet patient care needs.
The candidate should understand and accept that there are no guaranteed hours, and they must achieve the required weekly productivity points to meet evaluated weekly hours.
