Job Description
Job Title: Physical Therapist - Home Health
Job Specialty: Therapy/Rehabilitation
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of adult home health experience
License: Must have a current license in hand
Certifications: CPR, Basic Life Support (BLS)
Must-Have:
- Reliable transportation
- Current driver's license
- Agency-required liability insurance
Job Description:
- Conduct home visits to provide therapy and rehabilitation services
- Attend periodic office meetings
- Average of 5-6 patients per day
- Points system for daily productivity, 30 points per week required
- Assigned between 5-10 patients per day, equating to roughly 6-8 points
- Types of cases include start of care and follow-ups
- Must be willing to travel up to 75 miles from the assigned clinic
- Mileage reimbursed following the IRS Standard rate, compensated from patient home to patient home
- Use Pointcare EMR system
- No weekend or on-call requirements unless otherwise discussed
- Must be open to floating to nearby locations if needed due to low patient census
- All documentation must be completed within 24 hours of the appointment
- Understanding of no guaranteed hours and flexibility to meet weekly productivity points of 30, which equates to a standard weekly workload
