Job Description
Job Title: Physical Therapist - Home Health
Job Specialty: Therapy/Rehabilitation
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum of 1 full year of Adult Home Health experience required
License: Must have a valid license in hand
Certifications: CPR, BLS
Must-Have:
- Reliable transportation, current driver's license, and agency-required liability insurance
- Experience with HomeCare HomeBase (HCHB) EMR system
- Proficiency in OASIS if previously applied in a related discipline
Job Description:
- Conduct home visits for therapy and rehabilitation services.
- Participate in periodic office meetings.
- Handle a daily patient caseload, typically 5-10 patients, equating to 6-8 points daily.
- Travel up to 75 miles from the home clinic to meet patient needs.
- Complete required documentation within 24 hours of each patient appointment.
- Be open to floating to nearby clinics if patient assignments are insufficient.
- Attend start of care and follow-up home health cases.
- Cover the specified territory including Poquoson, Newport News, Yorktown, and Hampton.
- Meet the weekly productivity expectations of 30 points, translating to approximately 40 hours weekly.
- Weekend requirements and on-call duties may vary; these should be discussed during the interview process.
- Mileage is reimbursed at the IRS standard rate for patient-to-patient commutes, but not for the commute to/from the first and last patient.
