Job Title: Physical Therapist
Job Specialty: Home Health Therapy
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum of 1 full year of adult home health experience
License: Must have a valid license in hand
Certifications: CPR, BLS
Must-Have: Experience with HomeCare HomeBase EMR within the last 6 months
Job Description:
- Perform home visits and periodic office meetings for a variety of cases, including start of care and follow-ups.
- Must have reliable transportation, a current driver's license, and agency-required liability insurance.
- Handle an average of 5-6 patients per day, with a point system determining productivity (30 points per week).
- Conduct patient visits in both care facilities and patient homes.
- Proficient use of HomeCare HomeBase and Oasis for documentation; all documentation must be completed within 24 hours of the appointment.
- Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the standard rate.
- Be prepared to discuss on-call rotations and weekend requirements during the interview.
- Be open to floating to nearby clinics if the patient census is low to meet financial needs.
- Participate in weekly meetings and report to the designated location.
- Maintain a flexible approach to scheduling and location demands as patient needs require.
- Be comfortable accepting patients regardless of travel distance and ensuring timely, complete care delivery.
