Job Description
Job Title: Physical Therapist - Home Health
Job Specialty: PT Home Health
Job Duration: 13 Weeks
Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum 1 full year in Adult Home Health
License: Must have a current and valid PT license
Certifications: CPR, BLS
Must-Have: Reliable transportation, current driver's license, and agency-required liability insurance
Job Description:
- Conduct home visits for patient care and attend periodic office meetings.
- Be responsible for driving to patients' homes daily and to the clinic for weekly meetings.
- Manage an average of 5-6 patients per day with productivity expectations of 28-30 points per week.
- Complete all documentation within 24 hours of patient appointments.
- Must be comfortable traveling up to 75 miles from the home clinic.
- Be open to floating to a nearby clinic if patient census is low.
- Experience with HomeCare HomeBase EMR required; proficiency must be demonstrated or a retraining course completed.
- Must be familiar with handling cases involving Start of Care and Follow-Ups.
- Must be open to on-call rotations and able to adapt to varying weekend shifts.
- Address patient care assignments efficiently, accepting every scheduled patient regardless of distance.
- Drive mileage reimbursed at IRS Standard rate from patient home to patient home.
- Must not be a current direct employee of any related locations if applying as a traveler.
