Job Title: Physical Therapist
Job Specialty: PT Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of Adult Home Health experience
License: Must have a license in hand unless it takes a few days to obtain
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase EMR and recent experience using the system within the last six months
Job Description:
- Conduct home visits for patient care and attend periodic office meetings.
- Manage an average of 5-6 patients per day, converting to approximately 6-8 points based on activities such as documentation and travel.
- Travel up to 75 miles from the home clinic and receive mileage reimbursement based on IRS standards.
- Engage in various case types including Start of Care and Follow-Ups.
- Responsibly handle driving to patients' homes and adhere to schedule expectations.
- Complete all documentation within 24 hours following patient appointments.
- Be adaptable to float to other nearby clinics if patient census does not meet financial needs.
- Cover a territory with a focus on home health within a specified 50-mile radius.
- Use HCHB EMR systems effectively for patient care management.
