Job Description
Job Title: Physical Therapist

Job Specialty: Home Health

Job Duration: 13 weeks

Shift: Day 5x8-Hour (08:00 - 16:30)

Guaranteed Hours: Facility can call off up to one shift per pay period

Experience: Minimum of 1 year of skilled home health experience

License: Pending license accepted

Certifications: BCLS

Must-Have:
- Reliable transportation
- Oasis experience

Job Description:
- Provide physical therapy services in home health settings.
- Visit up to six patients per day with a 1:1 patient ratio.
- Maintain productivity standard of 30 points.
- No weekend or on-call requirements.
- Must float between all facilities within the state.
- Expected to have own transportation with occasional maximum radius of over 50 miles.
- Chart using EPIC/Oasis systems.
- Complete required orientation and non-billable modules pre-start and annually.
