Job Title: Physical Therapist Home Health

Experience: 1-2 years preferred
License: State License required
Certifications: Basic Life Support (BLS) required
Must-Have: COVID vaccination required, religious and medical exemptions accepted
Description:
This position requires a Physical Therapist for a Home Health setting.
The shift is day-based with five 8-hour shifts per week.
The therapist will be assigned 8-12 adult patients.
First-time applicants are accepted.
Weekend work is not required.
On-call duties are not required.
Floating within the scope to meet facility needs is applicable.
Block scheduling is not guaranteed.
Mileage reimbursement is provided at the federal rate.
Compliance document deadline is 10 days prior to start.
No locals within a 75-mile radius are accepted.
A state license can be pending at the time of submission.
Travelers may need to take additional fingerprinting for compliance.
For non-local applicants, a valid driver's license or a transportation plan is required.
Flu vaccination is required with medical or religious exemptions allowed.
Orientation will include module completion and a potential MOAB certification course.
A second set of fingerprints will be required on arrival if traveling to homes with children.
If MOAB certification is not already obtained, it will be provided during orientation.
Productivity points and standards for Physical Therapists: 27 points for a 40-hour week.
Ensure all compliance information is submitted timely for a smooth onboarding process.
