Job Description
Job Title: Home Health Physical Therapist
Job Specialty: PT Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of Adult Home Health Experience
License: Must have active license
Certifications: CPR, BLS
Must-Have:
- Recent Home Health experience
- Reliable transportation, valid driver's license, and auto insurance
- Proficiency in HomeCare HomeBase EMR
Job Description:
- Conduct home visits for therapy and rehabilitation.
- Participate in periodic office meetings.
- Manage an average of 5-6 patients per day.
- Handle Start of Care and Follow Up cases.
- Travel up to 75 miles from the home clinic; mileage reimbursed at IRS standard rate for patient-to-patient visits.
- Complete all documentation within 24 hours of patient appointments.
- Meet weekly productivity expectations of 30 points (equivalent to roughly 40 hours of work per week).
- Be prepared for the possibility to float to nearby clinics if patient census is low.
- Familiarity with OASIS is required, with retraining necessary if not used in the last 6 months.
- Local candidates accepted at the same rate.
- Open to on-call rotations as discussed during interviews.
- Cover approximately 100 miles per week and participate in home health services in the specified region.
