Job Description
Job Title: Physical Therapist (PT) - Home Health
Job Specialty: Home Health
Job Duration: 13-week contract
Shift: Day 5x8-Hour (8:00 AM - 5:00 PM)
Guaranteed Hours: None
Experience: Minimum 1 full year of Adult Home Health experience required
License: Must have an active license prior to starting the assignment
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase EMR
- Experience with OASIS system within the last 6 months
- Reliable transportation with a valid driver's license and agency-required liability insurance
- Willingness to travel up to 75 miles from the home clinic
Job Description:
- Conduct home visits to provide therapy and rehabilitation services to patients.
- Attend periodic office meetings as required.
- Carry out Start of Care and Follow Up appointments.
- Manage an average of 5-6 patients per day.
- Complete and submit documentation within 24 hours of each appointment.
- Ability to travel between patient homes, with mileage reimbursed at the IRS standard rate for distances traveled during work hours.
- Possible weekend on-call requirement, typically once per month.
- Be flexible to cover both patient homes and care facilities.
- Reach productivity expectations of 30 points per week, with each visit and activity holding different point values.
- Engage with patients both in their homes and possibly in care facilities.
- Consideration for hire at the end of the contract.
- Attend weekly meetings at the designated location and must be comfortable with daily drives potentially longer than 20 minutes between patients.
- Be prepared to float to nearby clinics to pick up hours if needed due to low patient census.
- Must accept all patient assignments regardless of distance to maintain efficiency and productivity.
