Job Title: Physical Therapist
Job Specialty: PT Home Health

Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not Guaranteed
Experience: Minimum 1 full year Adult Home Health Experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase EMR
Job Description:
- Conduct home visits for therapy and rehabilitation services.
- Attend periodic office meetings.
- Must possess reliable transportation, a current driver's license, and required liability insurance.
- Handle a daily average of 5-10 patients with workload equating to 6-8 points based on visit and activity types.
- Willingness to travel up to 75 miles from the home clinic.
- Mileage reimbursement at the standard rate for patient-to-patient travel; commuting time is not billable.
- Types of cases include Start of Care and Follow-Ups.
- Proficiency in OASIS and HomeCare HomeBase EMR required.
- Complete all documentation within 24 hours of the patient appointment.
- Be open to being floated to nearby clinics if patient census is low.
- Participate in on-call rotations if required, varying by location.
- Compliance with weekly productivity expectations of 28-30 units.
- Applicants must not currently be employed directly by any affiliated locations.
