Must-Have: Experience in Home Health, reliable transportation, current driver's license, liability insurance

Job Description:

- Conduct home visits and periodic office meetings.
- Maintain a patient load of 5-6 patients per day, achieving productivity expectations of 30 points per week.
- Travel up to 75 miles from the home clinic, with mileage reimbursement at IRS Standard rate.
- Start of Care and Follow-Up patient cases.
- Use of PointCare EMR for documentation.
- Complete all documentation within 24 hours of the appointment.
- Be open to occasional on-call rotations and weekend work, as required.

Please note that this position has non-guaranteed hours and requires flexibility in accepting patients assigned, regardless of distance. Detailing and timeliness in documentation are crucial to the role.
