GENERAL SUMMARY/BASIC PURPOSE OF JOB:
The Clinical Pharmacist Specialist provides comprehensive pharmaceutical care to patients through
integrated drug distribution, clinical services, research and teaching. The specialist acts as a leader on
the care team, pertaining to the optimization of drug therapy - in this case, at discharge -- to achieve
positive patient outcomes - in this case, beyond the current hospital stay. Key in ensuring the optimal
provision of pharmaceutical care by the specialist is collaboration with health care professionals and
integration into the area care team.
The Transition of Care Pharmacist, improves the transition from hospital to home for moderate to high risk
patients. The pharmacists objective is to reduce the 30 day readmission (due to medication-related
issues) of complex patients by executing a series of processes, including discharge medication
reconciliation, improving the accuracy of home medication lists, education on medication regimens to
increase compliance and counseling on any medication changes to the patients regimen during
hospitalization.
QUALIFICATIONS
Must possess a PharmD Degree.
Completed a PGY1 Pharmacy residency -- Pharmacy Residency (Pharmacy Practice,
Ambulatory Care or Community) or equivalent experience required (2 to 5 years in ambulatory or
transitional care environment)
Licensed as a pharmacist in the state of Illinois.
