Must-Have:
- Case Management experience
- Experience with Meditech and Midas

Job Description:
- Data Collection including discharge planning and care progression
- Communication with medical teams to facilitate patient progression
- Work alongside one Social Worker and one RN during the day shift (no case management on night shifts)
- Manage a patient ratio of 1:25
- Open-minded and proactive personality is preferred
- Rotation through weekends required, one weekend a month
- Participation in rotating holidays
- Ability to float to Med/Surg and ICU as needed
- Dress Code: Business casual or Navy scrubs
