Job Description:
- Collect and analyze patient data for case management.
- Perform discharge planning to ensure smooth patient transitions.
- Manage patient care progression effectively.
- Communicate with medical teams to coordinate comprehensive care plans.
- Adapt to a high patient ratio of 1:25 with efficiency and dedication.
- Work collaboratively with a team comprised of one RN and one social worker during day shifts.
- Display an open-minded and proactive approach to patient care and teamwork.
- Abide by the dress code: business casual or navy scrubs.
- Flexibility to work one weekend a month and rotating holidays.
- No requirement for floating to other units or night shifts.
