- Case Management experience
- Meditech and Midas experience

Job Description:
- Work in a unit with 36 beds and a team consisting of 1 RN and 1 Social Worker during the day shift. No case management on night shifts.
- Handle a patient ratio of 1:25 for RN.
- Utilize Meditech and Midas EMR for daily operations.
- Focus on discharge planning and patient progression.
- Engage in effective communication with medical teams for care progression.
- Adapt to floating across Med/Surg and ICU as needed.
- Adhere to dress code of business casual or navy scrubs.
- Participate in one weekend shift per month and rotating holidays.
- Exhibit an open-minded and go-getter personality to fit well within the team.
