Job Title: Home Health Registered Nurse (RN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: 8:00 AM - 4:30 PM or 9:00 AM - 5:30 PM
Guaranteed Hours: 36 hours per week
Experience: Minimum of 1-year acute care (Med/Surg) or home health care experience required
License: Registered Nurse (RN) - State Licensing Board
Certifications: Basic Life Support (BLS) - American Heart Association
Must-Have:
- Completion of Motor Vehicle Report
- Valid driver's license
- Proof of full coverage auto insurance with required limits
- Experience in Med/Surgical and/or home health care
- Proficiency with Epic EMR and Oasis assessment tool
Job Description:
- Perform comprehensive nursing care and develop care plans tailored to patients and their families, considering cultural, ethical, and spiritual needs.
- Conduct initial nursing evaluation visits to communicate with physicians and determine care plans.
- Collaborate with patients, families, and multidisciplinary teams to ensure optimal outcomes and enhance patient experiences.
- Assess, collect, and analyze data to determine nursing diagnoses, patient needs, and plans of care.
- Implement and prioritize care plans, coordinating care delivery while ensuring a safe, respectful, and ethical practice environment.
- Communicate timely changes in patient assessments and reassessments with providers and maintain communication with the multidisciplinary team for safe transitions and continuity in care delivery.
- Promote patient independence through education about conditions, medications, and self-care, answering related questions.
- Maintain a safe work environment by adhering to infection-control policies and protocols.
- Ensure equipment operations by completing preventive maintenance and managing nursing supplies inventory.
- Supervise clinical care provided by LPNs and Home Health Aides.
- Participate in an on-call rotation and provide patient care outside regular business hours.
Physical Demands & Working Conditions:
- Frequent travel required
- Must be able to meet physical demands such as standing, walking, sitting, lifting, carrying, pushing, pulling, stooping, kneeling, and crouching
- Ability to work in various environmental conditions
