Job Description
Job Title: Level 2 Case Manager RN

Job Specialty: Registered Nurse Case Manager

Job Duration: 13 Weeks

Shift: Monday to Friday, 8-hour shifts, Day shift 8:00am-4:30pm, weekends as needed

Guaranteed Hours: Not specified

Experience: Minimum one year as a clinical nurse in a hospital; 9 months experience in home care preferred

License: RN License (NY License ONLY)

Certifications: BLS, ACLS (AHA ONLY)

Must-Have:

- Certified Home Health Care experience
- EPIC or Netsmart preferred
- Oasis experience required
- Current NYS driver's license

Job Description:

- Develop and revise plans of treatment and discharge plans in collaboration with patients, families, physicians, the care delivery team, and third-party payors.
- Obtain medical orders and approvals for medical services.
- Complete and submit OASIS assessments, reassessments, transfers, care resumptions, discharges, and significant change assessments within defined time frames.
- Process orders and notify physicians of patient needs and changes in condition.
- Complete certification/recertification orders and discharge summaries.
- Provide a respectful environment that ensures patient privacy, confidentiality, and respect for property.
- Initiate appropriate preventive and rehabilitative strategies.
- Provide services requiring advanced nursing skills.
- Consult with and educate patients, families, and team members on disease processes, self-care techniques, and preventive strategies.
- Supervise LPNs to ensure patient health, safety, and compliance with the plan of care.
- Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the Home Health Agency.
- Interact with patients, physicians, referral sources, and others to maintain positive relationships.
- Support care management philosophy, promoting customer-focused planning and case management to achieve quality and cost-effective outcomes.
- Complete all clinical records in compliance with state and federal legislation, ensuring seamless service delivery.
- Maintain current knowledge of available services.
- Demonstrate flexibility and willingness to visit patients based on need.
- Resolve problems at the point of contact.
- Ensure authorization and release of information forms are signed by patients or responsible parties.
- Attend patient care conferences for assigned patients.
- Monitor results and progress, addressing quality indicators and customer satisfaction.
- Respond to quality management reviews/audits and resolve potential problem areas.
- Participate in peer review and quality assessment and performance improvement.
- Maintain productivity and ensure appropriate visit authorizations are in place, with visitation documentation reflecting the medical necessity for care in the home setting.
- Contribute to an environment of professional growth, learning, trust, and mutual respect.
- Promote recognition and respect for the diversity of the workforce.
- Participate in education, in-service training programs, and self-development.
- Demonstrate commitment to professional growth and competency.
- Responsible for performance indicators as established by management.
