Job Description
Job Title: Level 2 Case Manager RN

Job Specialty: Registered Nurse Case Manager

Job Duration: 13 weeks

Shift: Day shift, Monday - Friday, 8:00 am - 4:30 pm; weekends as needed

Guaranteed Hours: Not specified

Experience: Minimum one year of experience as a clinical nurse in a hospital required, and 9 months experience in home care preferred

License: RN License (NY License ONLY)

Certifications: BLS, ACLS (AHA ONLY)

Must-Have:
- Certified Home Health Care Experience
- EPIC or Netsmart Experience Preferred
- OASIS Experience

Job Description:

- Develop and revise plan of treatment and discharge plan in collaboration with patient, family, physician, care team, and third-party payors.
- Complete and submit OASIS assessments and documentation in accordance with agency timelines.
- Notify physician of patient needs and changes in condition and complete required orders and summaries.
- Maintain respect for patients' privacy and confidentiality.
- Initiate preventive and rehabilitative strategies and provide specialized nursing services.
- Educate patients, families, and team members on disease process, self-care, and preventive strategies.
- Supervise LPNs to ensure patient safety and compliance with care plans.
- Coordinate, evaluate, and supervise quality of patient care services provided by the agency.
- Support Care Management philosophy focusing on quality and cost-effective outcomes.
- Ensure completion of clinical records in compliance with state and federal legislation.
- Demonstrate flexibility and willingness to visit patients based on need.
- Resolve issues at point of contact and ensure necessary authorizations are obtained.
- Attend patient care conferences and participate in achieving agency operational and financial objectives.
- Monitor quality indicators and customer satisfaction; pursue reduction of costs and errors.
- Respond to Quality Management reviews and participate in peer review and performance improvement initiatives.
- Maintain productivity requirements and ensure documentation supports medical necessity for home care.
- Contribute to a professional growth environment, promoting diversity and mutual respect.
- Participate in education, in-service training, and self-development.
- Responsible for Key Performance Indicators established by management.
- Perform other duties as requested.
