Job Description
Job Title: Level 2 Case Manager Registered Nurse

Job Specialty: Case Manager RN

Job Duration: 13 Weeks

Shift: Monday to Friday, 8:00 am - 4:30 pm, Day shift

Guaranteed Hours: Not specified

Experience: One year of experience as a clinical nurse in a hospital required, and nine months of experience in home care preferred

License: Registered Nurse License (NY License ONLY)

Certifications: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS - AHA ONLY)

Must-Have:
- Certified Home Health Care Experience
- EPIC or Netsmart experience preferred
- Oasis experience required
- Current NYS driver's license

Job Description:
- Develop and revise treatment and discharge plans with the patient, family, physician, care team, and third-party payors, including obtaining medical orders and approval of services.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within defined timeframes.
- Process orders and notify physicians of patient needs and condition changes. Complete certification/recertification orders and discharge summaries.
- Provide an environment promoting respect for patients, their privacy, confidentiality, and property.
- Initiate appropriate preventive and rehabilitative strategies.
- Deliver services requiring substantial specialized nursing skills.
- Consult with and educate patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies.
- Supervise LPN to ensure patient health, safety, and adherence to plan of care.
- Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the home health agency.
- Foster positive relationships with patients, physicians, and referral sources.
- Support care management philosophy, focusing on collaborative, customer-focused planning, and case management.
- Ensure completion of clinical records needed for compliance and seamless service delivery within established timeframes.
- Maintain current knowledge of community-based services.
- Demonstrate flexibility and willingness to visit patients based on need, not geography.
- Resolve issues at the point of contact.
- Ensure authorization and release forms are signed by patients or responsible parties.
- Attend patient care conferences for assigned patients.

Contribute to achieving the agency's operational and financial objectives by monitoring results and progress, taking corrective action, responding to Quality Management reviews/audits, maintaining productivity, and ensuring appropriate visit authorizations. Promote a professional growth environment and participate in education, professional development, and self-development. Perform other duties as requested and demonstrate commitment, growth, and competency.
