Job Title: Level 2 Registered Nurse Case Manager
Job Specialty: Case Manager RN
Job Duration: 13 weeks
Shift: Monday - Friday, 8-hour shifts, Day shift 8:00 am - 4:30 pm (weekends per need)
Guaranteed Hours: Not specified
Experience: One year of experience as a clinical nurse in a hospital required, and 9 months of experience in home care preferred
License: RN License (NY License ONLY), Current NYS driver's license
Certifications: BLS (any provider is accepted)
Must-Have:
- Certified Home Health Care Experience
- Critical thinking skills, ability to follow standards/procedures, make appropriate clinical judgments to provide safe and effective client care
- Strong interviewing, clinical assessment, and communication skills
- Ability to exercise initiative and independent judgment
- Ability to build and maintain professional relationships
Job Description:
- Develop and revise plans of treatment and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payers, including obtaining medical orders and approval of medical services.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with agency-defined timeframes.
- Process orders and notify physicians of patient needs and changes in condition. Complete certification/recertification orders and discharge summaries.
- Provide an environment that promotes respect for the patients, their privacy, confidentiality, and property.
- Initiate appropriate preventive and rehabilitative strategies.
- Provide services requiring substantial specialized nursing skills.
- Consult with and educate the patient, family, and other team members regarding disease processes, self-care techniques, and preventive strategies.
- Supervise LPNs to ensure patient health, safety, and compliance with plans of care.
- Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the Home Health Agency.
- Interact with patients, physicians, referral sources, and others in a manner conducive to continued positive relationships.
- Support Care Management philosophy, including collaborative customer-focused planning and case management designed to meet individual health and service needs and promote quality and cost-effective outcomes.
- Ensure the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.
- Maintain current knowledge of all available community-based services.
- Demonstrate flexibility and willingness to visit patients based on need and not geography.
- Ensure problem resolution at the point of contact.
- Ensure authorization and release of information forms have been signed by the patient or responsible party.
- Attend patient care conferences for patients assigned to their care.
- Monitor results, progress, and take corrective action on quality indicators, including the level of customer satisfaction with the RN function.
- Understand the importance of the RN function and its impact on patients, other care providers, readmission, productivity, utilization, revenue, and expense.
- Pursue efforts to reduce or eliminate avoidable costs and errors.
- Respond to findings of various Quality Management reviews/audits and ensure resolution of potential problem areas.
- Participate in peer review and Quality Assessment and Performance Improvement as assigned.
- Maintain productivity requirements.
- Ensure that appropriate visit authorizations are in place and that visit documentation reflects the medical necessity for care to be received in the home setting.
- Promote the recognition, respect, and celebration of workforce diversity.
- Participate in education including orientation, in-service training programs, and self-development as needed.
- Demonstrate commitment, professional growth, and competency.
- Responsible for the Key Performance Indicators (KPIs) established by the manager.
- Perform other duties as requested.
