Job Title: Physical Therapist
Job Specialty: Occupational Therapist
Job Duration: 13 weeks
Shift: Monday - Friday, 8:00 AM - 4:30 PM; weekends as needed

Experience: Minimum of one year in Long Term Care/Home Care setting
License: Current state license in Physical Therapy
Certifications: BLS (AHA ONLY)
Must-Have: Recent Home Care experience within the last year
Job Description:
- Provide case management and direct patient care within a team environment.
- Administer comprehensive physical therapy modalities in the home setting.
- Assist the physician in evaluating the level of function and help develop the plan of care, revising it as necessary.
- Prepare clinical and progress notes and advise and consult with the family and other agency personnel.
- Participate in in-service programs.
- Participate and assure that care delivery services are consistent with the agency vision, mission, and strategic goals.
- Promote the agency's philosophy, mission, and administrative policies to ensure the delivery of quality care.
- Develop and revise plan of treatments and discharge plan with patient, family, physician, and care team.
- Provide supervision of Physical Therapist Assistants (PTAs) to ensure patient health, safety, and compliance with the plan of care.
- Consult with and educate the patient, family, and team members on disease processes, self-care techniques, and preventive strategies.
- Implement home therapy programs, including gait training, transfers, ADLs, home modification, and equipment use.
- Periodically evaluate patient's progress and make necessary adjustments to the physical therapy program.
- Instruct para-professional staff in body mechanics, equipment use, bed mobility, transfers, and home exercise programs.
- Interact with patients, physicians, and referral sources in a professional manner conducive to positive relationships.
- Participate in regularly scheduled patient conferences to ensure comprehensive and coordinated plan of treatment.
- Assure the completion of clinical records needed for state and federal compliance.
- Maintain current knowledge of all available community-based services.
- Demonstrate flexibility, enthusiasm, and cooperation in working with customers and team members.
- Ensure problem resolution and make constructive recommendations for improvement.
- Promote recognition, respect, and celebration of workforce diversity.
- Participate in professional organizations, continuing education, and share knowledge with team members.
- Responsible for achieving Key Performance Indicators (KPIs) established by the manager.
