Job Title: Physical Therapist

Job Specialty: Occupational Therapist

Job Duration: 13 weeks

Shift: Monday - Friday, 8:00 AM - 4:30 PM, with weekends as needed

Experience: Minimum of one year in Long Term Care/Home Care setting as a Physical Therapist; Recent Home Care experience within the last year

License: Current New York State License in Physical Therapy

Certifications: BLS (American Heart Association only)

Must-Have:
- Home Health experience within the last year

Job Description:

- Provide case management and direct patient care within a team environment.
- Administer comprehensive physical therapy modalities in the home setting.
- Assist physicians in evaluating the level of function and help develop the plan of care, revising it as necessary.
- Prepare clinical and progress notes, and advise and consult with the family and other agency personnel.
- Participate in in-service programs and assure that care delivery services are consistent with the agency's vision, mission, values, and strategic goals.
- Promote effective working relationships and work collaboratively as part of the team to meet the goals and objectives.
- Utilize resources, supplies, and time efficiently to contain costs and increase the value of all services to the community.
- Develop and revise plans of treatment and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payors.
- Provide supervision of physical therapy assistants to ensure patient health, safety, and compliance with plan of care.
- Consult and educate patients, families, and other team members regarding disease process, self-care techniques, and preventive strategies.
- Implement home therapy programs including gait training, transfers, activities of daily living, home modification, care, and use of equipment.
- Evaluate patient progress periodically and make necessary adjustments to the physical therapy program.
- Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the home health agency.
- Participate in regularly scheduled patient conferences to ensure a comprehensive coordinated plan of treatment.
- Cooperatively assist in the orientation of new rehabilitation personnel.
- Assure the completion of all appropriate clinical records needed for compliance with state and federal legislation.
- Demonstrate flexibility, enthusiasm, and willingness to work with customers and others necessary for support.
- Ensure problem resolution and respond to customer needs in a manner that meets or exceeds requirements.
- Participate in the attainment of the operational and financial objectives and monitor progress on quality indicators, including customer satisfaction.
- Support public relations and marketing initiatives to the local community and encourage continuous improvement using Total Quality Management principles.
- Engage in education, participate in professional organizations, and pursue continuing education opportunities.
- Maintain productivity requirements and ensure practices meet agency performance competencies.
